Pathway HousePathway House is a clever hybrid of the best of health and social care.
This new social care service is designed to prevent unnecessary hospital admissions and enable those ‘stuck’ in secure services, a new care pathway.
The culture of Pathway House is to provide opportunities for positive risk taking.
The beauty of Pathway House is that whilst it offers a homely, relaxed atmosphere it allows the people we support to explore boundaries. In a safe and structured setting, people can realise their potential and gain maximum control over their future.
Pathway House is an important new step in our integrated care pathway of diagnosis, rehabilitation, residential care and supported living.
The people we support and staff have full access to our multidisciplinary team of Psychiatrists, Psychologists, Speech and Language Therapists and Occupational Therapists.
Its proximity to Milton Park Therapeutic Campus enables clinicians to take positive risks, as they know if an individual experiences a moment of crisis; the clinical team is there to provide support.
Staff’s specialist training enables them to gain a clear understanding of the individual and to deliver person centred support.
The people we support are coached to understand their diagnosis and develop coping strategies to manage their anxieties, which reduce the frequency and severity of challenging behaviours.
Its supportive, homely environment allows each person to build their confidence and gain real choice and control over their lives.
Staff support individual's to improve their living and social awareness skills which are further developed in the community.
