Making a call to Brookdale Care is the first step of the referral process. Our Referral Team is specially trained in Autistic Spectrum Conditions and has a thorough knowledge of our services. You will be able to discuss your client or family member in confidence with a professional who will become your key contact during the referral process. All clients receive a pre-admission assessment which will establish their needs, abilities, risks and identify which of our services we feel would be the most appropriate.
All pre-admission assessment requests must be made by a care professional responsible for the individual’s care. We are happy to arrange for families and care professionals to visit our residential and hospital services to meet our management and clinical teams.
Referral Process
The Referral Team will work with the individual’s referring team and family to ensure a smooth and successful referral and transition into the identified service. We understand that not every referral can be planned and that some people with ASC may present in a state of crisis requiring urgent admission to a place of safety, we will aim to provide a rapid, accessible and responsive service in these circumstances.
Admission Criteria
All clients admitted to Brookdale Care’s services will have a primary diagnosis of Autism or Asperger syndrome, or exhibit autistic traits and tendencies requiring confirmation by diagnostic assessment.
Clients come to Brookdale Care from a variety of care environments including residential schools, residential homes, parental homes, supported living and secure hospitals.
Brookdale Care is unique it its ability to provide specialist care pathways from secure hospital through to residential care and supported living services for adults across the autistic spectrum.
Because of the specialist care pathways we are able to support individuals aged 18 – 65, with a primary diagnosis of Autistic Spectrum Disorder. They may also be experiencing mental health issues, have a learning disability, a forensic history or demonstrating challenging needs. We provide assessment and treatment, confirmation of diagnosis and a review of medication as part of their person centred care and treatment plan.
People within our services may be experiencing difficulties understanding their condition, managing their anxieties, impulses and lack the communication skills to engage with their community.
The Therapy Team will work holistically with the individual, their family and care professionals to create a proactive care plan that will meet their individual needs, enable them to develop independent living skills, social awareness and anxiety reducing skills that will support them on their care pathway to living in the community.
