Care Pathways

Brookdale Care is unique in its ability to provide an autism specific care pathway.

 

This flow of services enables individuals to progress through the care pathway as their abilities increase and needs lessen.

Every client throughout Brookdale Care is supported by the Multi Disciplinary Team – this means we can support individuals with complex needs in residential care instead of a more secure placement.

We can also provide additional support and peace of mind for our residential and Supported Living clients.  If an individual experiences mental ill health that cannot be managed within a community environment we are usually able to transfer the individual to Milton Park Hospital and treat the client’s mental health crisis at residential costings.  In the majority of cases we are also able to reserve the clients’ residential bed until they are able to be discharged from the hospital.
