Therapeutic Intervention

Multi-Disciplinary Team

Every member of staff at Brookdale Care is experienced in working with people with Autistic Spectrum Disorders (Autism and Asperger syndrome).

We employ our own multi-disciplinary team, which includes:
• Consultant Psychiatrists
• Consultant Psychologists
• Speech and Language Therapists
• Occupational Therapists
• Nurse Specialists
• Nurse Practitioners
• Qualified Teachers
• ASD Link workers

All team members receive training in Autistic Spectrum Disorders (ASD) and managing challenging behaviour. They are committed to achieving successful outcomes by adopting a person centred approach. Every client has a care and treatment plan which is discussed and agreed with the person and is supported by the multi-disciplinary team. The aim is to provide therapeutic interventions and structured activities that are individually tailored to help each client achieve their full potential. Brookdale Care staff are creative in seeking solutions to negative behaviours and understand the importance of providing a safe and tolerant environment whilst actively promoting independence and positive experiences.

Multi-Disciplinary Team Services
The multi disciplinary team maintains a high level of involvement in the care and ongoing support of clients and work with them throughout their stay to help them understand their condition, reduce risk, develop social awareness and gain the independent living skills required to sustain their progress.

This involvement includes:
• Initial assessment and diagnosis
• Individual risk assessments
• Assessment and monitoring of mental health issues including full psychiatric and psychological assessment and comprehensive care plans
• Diagnostic confirmation based on continued observations and psychometric and clinical assessments
• Specific individual treatment plans follow evidence based therapeutic approaches
• Progressive multi disciplinary review of Individual care and treatment plans
• Speech and language assessments and development of appropriate communication strategies
• Occupational Therapy including assessment of Functional, Sensory and Independent living skills and development of person centred support plans
• Assessment of challenging behaviours and development of appropriate treatment interventions and care guidelines
• Development of Care Pathways aimed at promoting full community integration and involvement 
• Health promotion strategies and individual Health Action Plans
• Self medication assessment and support plan  
 
Therapeutic Input including:
• Cognitive behavioural therapy
• Socialisation and Rehabilitation strategies
• Psycho education including psychosexual development
• Understanding social roles, rules and boundaries
• Communication skills including alternative and augmented strategies e.g. PECS
• Social interaction skills
• Vocational and educational support
• Counselling
• Supported employment
• Close liaison with other professional involved in the patient's care
• Recommendations on future placements and assisting with transition plans
• Development of care pathways and transition to a suitable residential placement
• Life skills development
