Pathway House is an important step in our integrated care pathway of diagnosis, rehabilitation, residential care and supported living and is a clever hybrid of the best in health and social care.
This service is designed to prevent unecessary hospital admissions and enable patients in inpatient services to step down.
The beauty of Pathway House is that whilst it offers a homely, relaxed atmosphere it enables patients to explore boundaries. In a safe and structured setting, people can realise their potential and gain maximum control over their future.
Its proximity to Milton Park enables clinicians to take positive risks, as they know if an individual experiences a moment of crisis, the clinical team is there to provide support and continuity of care.
The staff’s specialist training enables them to gain a clear understanding of the individual and to deliver person centred support, which includes the ability to support people to improve their living and social awareness skills which are further developed in the community. This includes access to colleges, travel training and leisure opportunities.
Patients are supported to maintain the coping strategies to manage their anxieties or to continue to build on the therapeutic work that has been completed at Milton Park or their previous placement. They also have access to our multidisciplinary team of Psychiatrists, Psychologists, Speech and Language Therapists and Occupational Therapists.
Its supportive, homely environment enables each person to build their confidence and gain real choice and control over their lives.
A rare and unique opportunity for people with complex long term mental health conditions to stepdown into a supported residential service.
